Go beyond a service label

Instead of writing general service, record the work, mileage, date and provider. Note the symptom that prompted the visit and whether it has resolved or needs follow-up. Report the provider’s explanation and identify uncertainties rather than presenting your own diagnosis as fact.

Separate work from payment

Use both date and mileage
